The Official ICD10; DSM-IV and SOC6 from HBIGDA Point of View


Today, as defined by ICD10, DSM-IV and SOC6 from HBIGDA:
GID means a kind of psychiatric disorder (DSM-IV), we know as transsexualism, understood as a lack of perception of sex, or genital reality, having feelings or desires to be of the opposite sex. That situation would generate a mental suffering, derived from that inhability to understand reality properly.

GIDNOS means a non-transsexual GID considered as GID-Not Otherwise Specified.
So, for those official definitions:

1st: GID includes Transsexuals(F64.0 and F64.2) and Crossdressers (F.64.1);

2nd: GIDNOS includes transgenders, and sometimes not so well sex reassigned intersexuals and hermaphrodites, and any other not so well defined situation (F64.8).

3rd: All those situations may be considered a Gender Dysphoria, the best "umbrella term", to define all those situations.

4th: Officially the World Health Organization-WHO, through the ICD-10 (International Disease Code) consider a GID (transsexuality and crossdressing) and all GIDNOS as mental disorders (based on DSM-IV).

The Revision Gendercare Suggests


For Gendercare Gender Clinic, we consider all that classification , but we would like to suggest some revisions

1st: Knowing all GID (transsexual kind, F.64 and F.64.2) is derived from a discord between mind (derived from brain, and basal brain differentiation) and genital differentiation, and not only nor mainly from social relations (we have enough scientifical data that show that reality, from alive humans, dead humans, non-human primates, and other mammals- more than 40 years of consistent research neuro-endocrino-genic results, and a lot of case reports and books showing exactly that unexpected reality).

2nd: Knowing also all GID crossdresser kind (F64.1) is not derived from a brain/genital discord, but from a social problem, probably derived from a childhood trauma.

3rd: Considering trangenders would be better classified as a GID class between F64.0 and F64.1, between transsexuals and crossdressers, because as transsexuals and crossdressers they live a very well defined GID (we have in Brazil a lot of transgenders, with very typical and well determined feelings, stories and behavior) and also they are not, surely, transsexuals nor crossdressers, because they have their particular characteristics, and their origin probably is related to very deep and very early traumas, mainly mother unconscious primary rejections, for example.

4th. Considering almost all intersex or hermaphrodites, when assigned sexually as babies and children,they do not feel rejected, but when they show discordant gender identities... they feel mainly as transsexuals... and soon desire SRS and complete sexual reassignment... we suggest there could have happened a radical brain/genital inner discord... so , in that situation, they really are very well classified as GID F64.0 people, derived not from nature, but from surgeries, and not as GIDNOS, but as GID. As today intersex not so well assigned children could not be considered as having a mental problem, DSM-IV do not consider those situations as GID, but possibly as GIDNOS... but at DSM-IV times, remember, those discords would never happen... because the rearing properly of the child would be able to prevent those GID possibilities.

5th. We suggest, from our point of view, all those situations are well and better defined as GID and only very few among them could be classified as GIDNOS.

6th.We suggest also there are a lot of GID with no mental disorders (most part of them) and only some of them may have a mental disorder.

7th.We suggest also, mainly true GIDNOS may probably have problems derived from a true mental problem (perhaps a problem that could be well defined as an auto-gyne-phyllic or sometimes an auto-phalo-phyllic disorder). We suggest that classification in the hope it could be good for the understanding of the few GIDNOS cases, in the sense we define it here.

8th. We conclude that the GID/GIDNOS human being, living in our so ideological, taboo promoting and adoring, and religion-related societies, following their social sufferings, they may start generating a mental problem... problems derived from social exclusion, low self-esteem and misery.

8th: Even living that difficult social situation, there is no relation between a GID and mental problems...surely most part of GID have no relation with mental problems.

10th: I suspect we, as clinicians, therapists, counselors and authorities, we create a social distortion of REALITY, and not them. Perhaps they simply perceive their simple realities "as is", a kind of reality we, with our values and points of principle, could not easily understand... and as authorities, with our PhD's, we say they do not adapt properly to REALITY.
I would like to ask....WHAT REALITY they cannot properly understand?
They do not accept not REALITY but surely OUR REALITY, or what WE DEFINE AS REALITY!
Perhaps they have no real mental problem.... but probably our society, intending to mix up relative social realities as absolute realities, is showing some social and ideological very dangerous lack of ontological consistency.Doing that way, we are showing our mental weaknesses, and lack of logical and ontological criteria.About sex and gender, we think today as if we were in the Antiquity, in the times of Aristotle... the time of the absolute truths... we forget Kant... we forget Hegel... we forget Poincaré...

But, for all GID and GIDNOS patients, surely, we need to research their mental state to be sure there is not a mental problem association, before any hormone therapy, cosmetic surgery or SRS referral letters.
